Those germs can be virus, bacteria and fungi, which can be acquired from direct contact with inanimate objects we touch, such as door handles, stair railing and from person to person. Hand is the most potential way to spread the harmful germs; they enter our body by us touching our eyes, mouth and nose after being in contact with them. They can lead to serious bacterial infectious diseases such as the common cold, flu, and foodborne illnesses. Hand washing is the primary, simplest and most effective way to limit the spread of germs and infectious diseases. People clean their hands by using regular soap, antimicrobial soap and alcohol based hand rubs or sanitizers. Regular soap are detergent based product, in the form of bar, leaflet, tissue and liquid. They do not kill the germs; they clean by physically removing dirt, organic substances and contaminating organisms. Soap is mostly effective against loose transient organisms. Antimicrobial soaps are detergent based products with antiseptic agents which are intended to mechanically and chemically kill all transient and resident bacteria that can cause illnesses by their long term lasting action. They inhibit the growth of virus. Alcohol based hand sanitizers come in the form of gel or rub cloth. They contain 60%–95% ethanol or isopropanol. Alcohol based hand sanitizers have a broad spectrum activity; they inhibit the growth of most clinically important viruses, yeasts, and fungi. According to the CDC, in vitro testing proved that Alcohol sanitizers are effective even against some resistant bacteria such as mycobacterium tuberculosis and certain envelop virus such as herpes simplex virus, human immunodeficiency virus (HIV). My hypothesis is that Alcohol based hand sanitizers are more effective than regular soap and antimicrobial soap. The regular soap do not kill the bacteria it only physically remove the loose transient bacteria, therefore there can still be a chance for the resident bacteria to cause illnesses. Even though the antimicrobial soaps kill both the transient and the resident bacteria, the frequent exposure of those bacteria to the antimicrobial agents may cause them to become resistant to them. It will give rise to more resistant microorganism strains. Alcohol sanitizers have the fastest bactericidal effect. It is effective against gram positive and negative bacteria even some of the most resistant ones.

In this assignment, I will reflect an incident happened during my clinical placement to develop my communication skills not just theoretically but in practical facing real life environment. There is a lot of factors that may influence nursing practice. In this essay, I will discuss the importance of communication in developing nurse-patient relationship. Communication in nursing as stated by Sheldon (2004) was, â€Å"Many definitions describe [communication] as a transfer of information between a source and a receiver. In nursing, communication is a sharing of health-related information between a patient and a nurse, with both participants as sources and receivers. The information may be verbal or nonverbal, written or spoken, personal or impersonal, issue-specific, or even relationship-oriented, to name a few possibilities. â€Å". The aim of this essay is to reflect what had happen during my placement and this gives me opportunity to improve my nursing care plan, which is based on the patient’s initial valuation. David Kolb,1984 said in his book called † Experiental Learning: Experience as a Source Of Learning and Development,(Kolb, 1984)† that reflection is a way in which we examine our experiences and draw lessons from them. QIA Key Skills Support Programme(2007) added that reflection can help to bridge the gap between theory and practice and enable us to look critically at our own behaviour, the behaviour of other people and at the organizational and social context within which we operate. In my context with my event, it is important for me to improve my nurse-patient relationship in terms of communication. In this reflection essay, I am going to use Gibbs (1988) model of reflection. This model is a recognized framework that will help me to reflect what was going on during my 16 days of clinical placement in one of major government hospital. Gibbs model of reflection consist of six stages where the stages link with each other making a continuous cycle to facilitate critical thought, relating theory to practice where this model allow me to see my weaknesses and improvements during my clinical placement. Gibbs’ model of reflection encourages you to think systematically about the phases of an experience or activity, and you should use all the headings to structure your reflection (Oxford Brooks University, 2011). This model of reflection also a good brainstorming of experiences that will end up with solutions to be made and things to be fix and for us to be aware in the future. Jasper M(2003) explains in her book called † Beginning Reflective Practice – Foundations in Nursing and Health Care† that Gibbs model of reflection starts with description of the event which I will discuss in my reflection. The second stage is going to be the analysis of feelings, where I have to tell how I am feeling when that situation happened. Third stage of Gibbs is evaluation of the experience, then comes the fourth stage that is analysis. In this stage I will discuss about the event and from what sense that I can use out of that situation that I am facing in the event. Conclusion and action plan are the last two stages which later on will discuss what else could I have done and what action can I do if the situation arose again along my course in nursing and in future time. Description In this paragraph, I would like to describe what happen during my clinical placement. I was selected to be in the female surgical ward in one of the government major hospital for my first clinical placement. The ward dealing with pre-operative, post-operative patients and orthopedic patients. The ward is divided into four cubicles which each of the two controlled by different unit; operative care unit and orthopedic unit. So, came this morning when me and my colleagues join the qualified nurses do their passing patients report, a family member of a patient name Ms. A approached me and complains that her daughter felt pain at her abdominal area. Ms. A is a 22-year-old young lady who has been diagnosed with colorectal cancer, (colorectal cancer occurs when tumors formed in the lining of the large intestine, also called the large bowel – National Cancer Institute,2003) as a result, giving her cramp feeling in the abdominal area, vomiting and change in bowel movements. She is in a nursing special care called palliative care,(palliative care – an approach that improves the quality of life of patients and their families facing the problem associated with life-threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychosocial and spiritual – WHO 2011) as her cancer was in incurable stage. I, as the student nurse immediately followed my mentor with few qualified nurses as they heard Ms.  A who was screaming continuously as she claims her pain was unbearable. It was my first time seeing or even hearing someone in that much pain, so I stood near her bedside and just look at the qualified nurses assess her after I introduced myself as student nurse. Effective communication is vital, as what have been stated in NursingTimes(2007) † Failure to communicate well with a patient right away will destroy the delicate n urse/patient relationship and mean the patient does not trust the nurse. † Feeling Here, I would discuss the feeling and thinking that I had when I was in the situation. First I introduced myself to Ms. A as I need to build a good rapport with Ms. A so she won’t feel awkward and panicked with my presence with other qualified nurses. Like Crellin. K (1998) said in her 11 Ways To Built Rapport article, â€Å"Taking time to establish a rapport with them can help them cope and improve their willingness in cooperate with the treatment†. I was shocked and stood near her as she can’t stand with her abdominal pain. There is limited skills that I can do to help Ms. A during that time as I am still new in the environment. I were taught to ask Ms. A condition and asisst her to calm down by communicating with her. Evaluation In my evaluation, I feel that I did the right thing to attend Mrs. A and help the qualified nurses dong their task by communicating and be an empathic listener (emphatic listener is a way of listening and responding to another person that improves mutual understanding and trust, Richard. S 2003) and active listener so that she feels safe and know that someone care about her other than her family. As said by Richard Salem(2003), Emphatic listening will build trust and respects and creates safe environment that is conducive to collaborative problem solving. So, these control her emotionally to make her to calm down and get back to her stable condition. The doctor who attend her indirectly taught me how to make Ms. A to calm down, by facing a challenging speaker who was in pain. The key to active listening is relaxed attention – to listen with my whole body by using verbal and non-verbal skill like facing the speaker(patient) and maintaining eye contact with them (University of Maine,1914). Analysis To analyse of what happened, the factors that affect my communication with Ms. A was her unbearable pain. Effective communication and act as a professional were quite difficult as I never had a long conversation with her except for asking permission for assessing her vital signs which were temperature, blood pressure and breathing rate. Active listening also needed in this context as it help the speaker clarify what they are saying and makes them feel heard and it reduces emotions that block clear thinking (University of Miane – 1914), hence it reduces her pain mentally. By listening to her, rephrasing what I heard the patient say as patient won’t be receptive to your response until her thought ,ideas and feeling are communicated and understood(U. M,1914). Conclusion To conclude this, I am sure that communication is the most important skill to have as this will lead me to a successful nursing practice in any situation. Without effective communication, I won’t be able to establish a good rapport with Ms. A and her family. As said by Sheldon, (2004) â€Å"The power of creative and effective nursing care is strengthened by good communication skills. Patients share their stories, symptoms, and concerns by talking with us. Both the spoken word and the body language convey information about the patient’s experience. Your words can do so much: put a patient at ease, set up a productive relationship, and carry out interventions. There is no other skill that is used more in nursing than communication. † and Kacperek (1997)suggested that effective communication is dependent on the nurse’s ability to listen and utilize non-verbal communication skills. Ms. A feels thankful for our presence helping her with er pain by giving painkiller right after her mother called us. Action plan If the same situation arose again, I would like to get myself prepared with any circumstances that might happen, in my case, to communicate with patients in professional way without waiting for emergency situations. Research, asking tips for good rapport and effective communication are ways to solve my communication weaknesses. Without proper communication, it is difficult to understand patient’s need. Therefore, I would like to remind myself that, effective communication is the bridge towards successful and efficient nursing practice. Therefore in nursing practice, it is essential to have the knowledge of interaction, not just practical skills like bed making, vital signs and bed bathing but well prepared with good communication skill to face any situation that we might experience anytime. Langston Hughes Salvation Essay In Langston Hughes’ essay â€Å"Salvation,† the author recounts how his failure to â€Å"see† Jesus and be outwardly saved results in a deeper, more stirring revelation: that only he and not Jesus can save his soul. Although Hughes devotes much of his essay to parodying the salvation experiences and apparent hypocrisy of other church members, and he tells us that the church building is stuffy, uncomfortable, hot and boring, he abruptly changes his tone at the end. When he describes how he cried in bed from guilt at having lied about his salvation, the reader realizes that Hughes has indeed undergone a powerful spiritual awakening: he has been saved from his own hypocrisy. Hughes starts off his essay using apparent irony by saying he â€Å"was saved from sin when [he] was going on thirteen. But not really saved. † (Hughes 351). This leads us to believe that he is cynical about Christianity, and we should not believe he is about to undergo any real spiritual transformation. When he describes having attended Auntie Reed’s Baptist church when he was not even thirteen years old, we get the impression that he is not responsible for taking the experience seriously. At that age, children are impressionable and naive about religion, which they may not understand, because it offers protection at a time when they are already protected and forgiven by their parents for errors they make, or â€Å"sinful† acts. Hughes gives the impression that he is being forced to go to church because of social expectations. He is strongly influenced by his friend Westley, who does not hide his real reason for getting baptized, which is to get out of that hot, stuffy church and get on with his adolescence. Westley tells Hughes â€Å"God damn! I’m tired o’ sitting here. Let’s get up and be saved! † Hughes (351). Westley’s ironic and comical interjection aptly sums up Hughes’ view of how the salvation process works in the mind of an adolescent: just do it and get it over with to make your elders happy, because it’s all a game anyway. It is only when Hughes’ aunt Reed comes and sobs at his side and is summoned by the minister that Hughes begins to break down Hughes (351). In paragraph 11, Hughes states: â€Å"Now it was really getting late. I began to be ashamed of myself, holding everything up so long. I began to wonder what God thought about Westley who certainly hadn’t seen Jesus either, but who was now sitting proudly on the platform†¦. So I decided that maybe to save further trouble, I’d better lie, too, and say that Jesus had come, and get up and be saved†¦. So I got up† Hughes (352). Here, Hughes discovers that the mere physical act of rising in response to his summons has begun to change him internally. He may not yet â€Å"see† Jesus, but he certainly sees and feels the effects of what he has done: â€Å"Suddenly the whole room broke into a sea of shouting, as they saw me rise. Waves of rejoicing swept the place. Women leaped in the air. My aunt threw her arms around me. The minister took me by the hand and led me to the platform†¦.. joyous singing filled the room. † Hughes (352). To Hughes, Jesus may as well be in the arms of his beloved aunt as in the â€Å"God damn† of his cocky pal Westley. He finds salvation in the spirit of the moment, and it feels good, if not a little confusing. Towards the end of the story in paragraph 15, Langston begins to notice what he has done wrong and feels emotionally distraught and full of neglect in his actions. He has fooled everyone into believing that he had found Jesus Christ. The only reason why he went up to the podium in the first place was to seek a way out sitting on the pews all day at church being told by the church elders, you’ll be damned if you don’t repent and be baptized! etc. Later on that night, Langston felt not only pressured into doing this but a sense of self actualization that he wanted to truly find Jesus, but in the end he ended up not only hurting himself, but the rest of his family for lying the whole time about his true feelings and the reason why he was in bed crying that whole night. Hughes writes, â€Å"I couldn’t bear the fact that I had lied, that I had deceived everybody in the church, and I hadn’t seen Jesus, and that now I didn’t believe there was a Jesus anymore, since he didn’t come help me,† Hughes (352). This passage provides a vivid explanation of his outlook on those around him and how he betrayed himself into lying for the sake of God. Hughes’ sadness and lack of faith came from those around him who kept on pressuring him to accept Jesus Christ into his heart. In the end, he appears to have turned away from God because he didn’t believe, rather he looked on the outside and not from within being only that he was twelve and naive. In the admissions process, forms were sent out to all Price Waterhouse partners of whom there were 662. These partners then submitted their comments about the candidates. . Only â€Å"thirty-two partners, all male, responded about Hopkins.† (p. 5) The forms were then tabulated to achieve a statistical rating in order to determine if the candidate should be admitted to the partnership. Of the 88 candidates â€Å"Price Waterhouse offered partnerships to 47 of them, rejected 21, and placed 20, including Hopkins, on hold.† (p. 1) â€Å"Some candidates had been held because of concerns about their interpersonal skills.† â€Å"the Policy Board takes evaluations or a negative reaction on this basis very seriously,† even if the negative commen ts on short form evaluations were based upon less contact with the candidate than glowing reports on long forms evaluations based on more extensive contact. The policy board had however, recommended and elected two candidates "criticized for their interpersonal skills†. (p. 4) Approximately 1% of the 662 partners were women. Price Waterhouse â€Å"gave two explanations for this. One was the relatively recent entry of large numbers of women into accounting and related fields. The other was the success of clients and rival accounting firms in hiring away female potential partners.† (p. 4). The Great Fire of 1666 prompted the construction of two new streets to intersect city as well as the widening of many of London’s streets and lane. Six new bridges were added to cross the River Thames between 1750 and 1835 and toll roadways were created to pay for repairs to existing thoroughfares. Hansom cabs were first introduced as another form of transportation in 1834 while the first passenger railway appeared just two years later. Deep water basins were added to the river at this time to reduce congestion from shipping. The Thames Tunnel was opened in 1843 to provide another avenue of transport over the river. Victorian London witnessed a revolution in transportation with the arrival of omnibuses, the extension of the railways for passenger trains to completely traverse the city, trams (first horse-drawn, then electrified by the turn of the century) and tube railways. The first underground railroad was opened in London in 1884. It was the very first of its kind and a model for other metropolitan areas in the ensuring decades. Appendix B contains a map of London’s current underground system with its extensive labyrinth of passageways. The growth of the commuting labor force living in the suburbs prompted the construction of new trunk roads and the expansion of the underground system to accommodate them in the years prior to World War II. Subsequent bombings of the city during the war in the 1940’s would escalate redevelopment of its ancient roadways. World War II In the midst of the second World War in September, 1940, Germany instituted the Blitz, a sustained six-month bombardment of London which killed 20,000 residents. After the bombing ended in May, 1941, London became a â€Å"major centre of wartime production† before Hitler renewed his assault on the city in summer of 1944 resulting in 29,890 Londoners killed and 50,507 injured. Underground rail stations were even used as shelters and the tunnels pressed into use for weapons storage. While the negative effects of these bombings included reduction of the population and significant damage caused throughout parts of the city, the end of the war became the impetus for England to start replanning the layout of London and surrounding towns, roadways and public transport and urge relocation from overpopulated areas to the suburbs (see Appendix C for a map of areas affected by bombing). Without a federal transport authority to draw up plans for redevelopment of roadways, much of the work was undertaken by landowners and commercial developers whose interests it would best serve. Construction of an airport began in 1946. In 1947, the legislature passed the Town and Country Planning Act to redevelop the slums of the East End, which had long been largely ignored since the majority of wealthy citizens resided in the West End. Narrow and impassable roads in the area were replaced for a nearly complete revitalization. After years of austerity in the war, it was with great acclaim that Britain crowned their new queen, Elizabeth II, in 1952. Her coronation ceremony, complete with a drive through London in the ancient Gold State Coach, prompted a renewed interest in horse and carriages. The British Driving Society was formed soon thereafter to honor the ancient sport of driving various horse-drawn vehicles . Royalty used the Long Walk at Windsor to display their horsemanship skills while the general public was confined to the pathways of parks in the West End. While Queen Elizabeth began her reign, the last of the famous London trams was officially retired. The decade of the 50’s was one rife with innovation, however. 1953 saw the opening of the newly completed Heathrow Airport in London. The first commercial air service between London and Moscow was launched in 1957 and the city received its first parking meters in 1958. The initial section of the London-Birmingham Motorway opened in 1959, closing out the decade with a loud roar.
